Who can be admitted to Leeway?

Anyone 16 years of age or older, who has HIV/AIDS, and who needs 24-hour skilled nursing services is eligible for admission. Leeway offers IV therapy, wound care, physical therapy, occupational therapy and speech therapy, as well as supervision with medications and treatments. Leeway accepts residents receiving dialysis treatments and methadone therapy.

How does one apply to Leeway?

Referrals are accepted from hospitals, home care agencies, outpatient clinics, as well as from private physicians. An application is completed and sent to the Leeway Admissions Coordinator, 40 Albert Street, New Haven, CT 06511. All necessary medical information and required tests must be completed in order to be placed on the facility waiting list. Placement on the waiting list does not guarantee admission to Leeway; it indicates only that the potential resident will be considered for admission. If all requirements are met, the applicant is admitted in the order in which he or she appears on the list.

What is considered in making a decision regarding who will be admitted?

The Admissions Committee considers whether or not the level of services required by the applicant can be provided at Leeway at that particular time, taking into account the resources, personnel, equipment and services available at Leeway and the medical and psychosocial needs of the applicant. Also considered are any communicable diseases that may be present and the applicant’s need for isolation facilities. Sometimes, additional information may be requested from the applicant’s medical providers.

All applicants must have a source of payment available to cover the cost of care at Leeway. Sources of payment include, but are not limited to, Medicaid, Medicare, private insurance, or other sources of payment.
