Leeway has garnered a strong quality reputation of caring for frail and vulnerable populations, many of whom are living with multiple co-morbidities. Through funding and collaboration with Connecticut’s Department of Social Services, and in partnership with existing New Haven providers, Leeway will broaden and build relationships to collectively co-create a community-based services and support system. Using a multidisciplinary care team approach, patients and their caregivers in the home setting will be supported to achieve and maximize their health and well-being. The team will aim to foster sustainable improvements through the creation of a person-directed healthy living plan that integrates both medical and social needs. Evidence suggests that this team-based approach can optimize health with improved adherence and mental health outcomes.
Together we will achieve this outcome in five steps. One, by conducting a community needs assessment and project visioning in support of program design, service integration, and places of dynamic services development. Two, by implementing a wrap around case management approach aligned with evidenced-based practices. Three, by creating a training program for multidisciplinary staff, patients/ caregivers, and community stakeholders including prevention, tools/care interventions, cultural knowledge and sensitivity, positive psychology, and coaching and motivational interviewing. Four, by launching marketing strategies to support the Informed Choice Campaign and to leverage Leeway’s brand awareness. And, five, by designing a community living center with a sustainable business plan for development in 2017.
